City of Corvallis Ambulance Service offers patients a financial hardship / charity adjustment program; providing an opportunity to reduce the balance on their account if they apply and qualify for financial hardship.

You must complete the financial hardship application and attach the requested documentation; please return the application and documentation to our office within two weeks’ time.
